Insurance Guidelines and Payment Information
Payment is expected at time of service.
Spring Hill Physical Therapy & Acupuncture is a provider for the following insurance carriers:
- Blue Cross of California and its affiliates
- Blue Shield of California
- UFCW
- Mail Handlers
- Pensioned Operating Engineers
- Unicare¨
Medicare
Medicare patients must pay a $135 deductible each year, payable in January. Some private Medi-Gap insurances will pay for this deductible. If another insurance provider, including Medi-Cal, does not pay the deductible, it then becomes the responsibility of the patient to pay it in full. Medicare does not accept referrals from chiropractors.
Private Insurance
We are now contracted with the local Independent Practice Association (IPA) which serves the HMO community. Please call us at 530.274.2320 for specific plan information.
We will bill out-of-network insurance companies, but payment is usually made at a reduced rate, resulting in a higher co-pay for you. All co-pays are due at the time of treatment.
We will bill auto insurance for treatment of injuries sustained in a motor vehicle accident after we have determined that a patient has med-pay benefits available. However, any services not covered by the insurance company are the patient's responsibility. We do not bill third-party auto insurance carriers. We will do our best to assist with coverage information, however, please be aware that each patient must understand his or her own insurance plan. In all cases, payment is ultimately the patient's responsibility.
As a courtesy, we will bill your insurance company for you. Please be aware of the following:
- We will make every effort to determine your insurance benefits prior to treatment.
- The information provided to us by your insurance company is not a guarantee of payment.
- Patients are responsible for any amount not paid by their insurance plans. This includes deductibles, co-payments, and non-covered services or items.
- All co-pays are due at the time of treatment.
- You are ultimately responsible for all payments. Please contact your insurance provider if you have any questions about your insurance coverage and benefits.
Payment Plans
If necessary, we will be glad to set up a payment plan. If you need this service, please discuss it with our front-office staff prior to beginning treatment. We ask that you make your payments on or before the 10th of each month. Late payments are subject to a 5% rebilling fee.
Fee for Services
Payment is required when services are rendered. We accept Visa, MasterCard, local personal checks, and cash.
Cancellation
Our office requires notice of 24 hours in advance when you need to cancel an appointment. There is a $50 charge for missed appointments billed to you and not to your insurance company.
